Every so often I would hit an "excellent" shot on the stroke meter and the shot would project completely out of the way.  This usually occurs with  my irons, especially of the tees.  For instance, if I were hitting my approach about 160 yards away with minimal wind and a fairway lie with an "excellent" or practically near it stroke, my shot would shoot off dramatically to the left or right of where I was aiming.  Is anyone else experiencing this?  What am I doing wrong?  How do I overcome this ordeal?  Again, this happens randomly.

Looking forward to your comments and thanks for any helpful feedback.

Look around at the different club sets in the pro shop, higher rated clubs will spray offline less than the starters.  Precision means it will spray less on a perfect hit, forgiveness means it will spray less on a mishit shot.

You do realize there's sometimes strong crosswinds?

Ah, dude, read the post again: "...with minimal wind..."

Wups, sorry, I did miss that.  It's just that if I account for the wind, and click where I'm aiming, my shots don't "shoot off dramatically to the left or right of where I was aiming".

I have noticed that once in a while a 'phantom' club selection occurs. I think this is actually user error as it only happens after reviewing clubs. Or maybe not...

In the same way, maybe a phantom direction selection is made?

If using laptop, the touchpad is often sensitive to brushes and maybe moving about has caused the 'phantom' direction selection

Were you able to notice if the triangle was definitely still where you intended?

    Yes this is what happens

If you do not like then do another golf simulator that does not do this as your error is based on how close you were to the mark
